Truck Driver DAC Report – Request your copy or start the dispute process


As many of you know, we are beginning the process of submitting a request to Congress for an investigation into the practices of the DAC Reporting Services, and how trucking companies abuse this service by reporting false accusations against drivers.

If you would like to request a copy of your DAC report, you can begin by filling out the proper form listed below:

    why cant these forms be filled out online and submitted, you sind out your dac report is messed up from a vindictive company or dispatcher ussually when you are looking for work, you dont have 30 days and acsess to a printer /fax, and may fill forms out incorrectly, i mean whats required? i dont want to know about alot of stuff on that report just the dac. how come a company can pull it up and look at it in 5 minutes, but you have to mail in some form, whats that about?
